Old Equipment lease - Sundance Business VOIP Telephone Help

Does anyone know what the following is, I am trying to trace it down. All I have is
what I think is a model number 'E21CP96T'
and '200 DATA 9600BPS SR'.

I only have Lease number and the Model(I think) and description.

I will see if I can find out what system this belongs to.
